I would like to take time to send a shout out to Hollywood’s RN Manager. Since day one she has really been such a positive role model and really has helped us in the Nursing Dept. She is really helpful and a team player. She’s always busy helping us MA’s when needed. She has made work run smoother. She takes entitative and steps up to the plate when need. Monday we only had one MA for half the morning, she covered the providers meanwhile the MA was working phlebotomy and still helped with injections. It was a hectic Monday morning at Hollywood, and we managed to get it all done with no issues before lunchtime. We are extremely with her presence and just amazing help. She also listens to our needs and concerns. She advocates for her staff. It really has made such a positive and good environment in the nursing dept at HCC. I really hope she gets recognized for all her wonderful work and support. She advocates for her staff as well as the patients. She finds a way to help everyone when they have questions, concerns. Department: Business Development
Submitted by: Michel McVicker
Audrey identified a patient who had come in for treatment of an STD during Wellness at BEN who was on the 104 report from the WDC site and brought it to the attention of the BEN OA and MD. They then decided to draw his labs while he was in the office and schedule a follow up for him at the WDC site before he left the office. Because of her attention to detail, the team was able to wrap around the patient, deliver the care he needs to thrive, and educate the patient on why his appointments and medication are important to maintaining his overall health.

I accidentally got locked out of my office and Anyssa was the best. She went back and forth from OTC to HQ and up and down the elevator trying to get me back in. This is among watching her put out a million other little fires. She’s awesome. She was calm kind and collected as she helped me with this frustration, and instead of handing me off to a locksmith or the building manager she was fully involved. She also followed up to ensure I made it in ok. It’s inspiring to see someone working so hard to make other people’s lives a bit easier.

Shalrese has always helped me, even when it should have gone to other people. She is such a valuable employee, and I know that I am not the only person she gives this treatment too. Her customer service is always above and beyond and she is definitely a PARTNER when it comes to working with HR.
Nominee: Miyoshi LaFourche
Department: Department of Medicine/Healthcare Centers
Submitted by: Samera Owhadian
Representing AHF on my behalf, Miyoshi LaFourche took on the task of helping me apply for a new loan repayment program offered by the Dental State Board of California. Miyoshi made time to research the program, follow up with the Board as needed and communicate with the medical directors and HR the criteria for the program. She did this in a most timely manner and without hesitation. Miyoshi has been a constant source of support for me as a provider since the interview process, and I truly appreciate her efforts in helping me complete the application requirements. I am ecstatic that I have been chosen to be a recipient of the Board’s loan repayment award. This program will be life changing financially for me, and it would not have happened without Miyoshi. On many levels, I feel like the burden of student loan debt can be a big distraction to providers; with this additional repayment, I can focus more on providing excellent patient care. Whether or not I would have received the award, I am beyond thankful for Miyoshi’s efforts. She is always willing to listen and to lend a helping hand or guidance if possible, and that is invaluable to me.
Nominee: Houston
Department: Executive/Administrative
Submitted by: Bret Camp
I have listed this nominations as Human Resources because it was the human resources of everyone in AHF Houston that made the recent relocation, combination, and execution of the new Binz HCC patient centered model and a new PHD mega testing site successful. Everyone in Houston worked hard toward this goal, and when I look across the listing of business lines and divisions there is not one that did not touch or support this project. Local staff crossed business lines and assigned worksites to assist, lend a hand, and learn. Many had long days and came in over the weeked, Attitudes were can do and cheerful. The combined teamwork and attitude accomplished their goals with minimal down time. Patients did not miss services. Houston is able to serve more patients.

This project allows AHF to provide cutting edge medicine and advocacy regardless of ability to pay. It is patient centered, values employees, and respects diversity. We did what was right, and above all were nimble.

I cannot say enough times how thankful I am that she continues to go above and beyond for us. I have leaned on her many times, and she has never been unavailable to me, She continues to shine and do great things. The organization is lucky to have her.

MS. Sherry has really helped me in the areas of need such as regaining my self-esteem, confidence in myself, listening ear, making me feel comfortable that i can share things i need to say, open me up to talk about things that bothers me, gives me hope when i feel it is none. She is a great asset to the program, she goes beyond and doesn’t judge me. Help me with a clear view when i felt it was none, she doesn’t push me when I am not ready. Easy going along with no nonsense in a good way. She has helped me beyond what I am sure she is required to do. Also leave her with a since of pride. She has helped me to be more task driven. She is a true supporter of me and not just in it for financial gain. She has came to my home and never judged me afterwards. Goes out of her way to make sure I am OK after she has talked to me. I would not be where I am not if it wasn’t for her and her willingness to help. I want to see more people like Ms. Sherry. I Know her title is Case Manager but she is more like a Mentor and a I am on my way to being Virally suppressed and making sure that I go to all my schedule appointments now and in the future. I know know how to ask for what I need and where to do that. I don’t know where I would be without her and I am glad I now know who she is for the rest of my life. She is helping me with a Lifelong change to help me for the rest of my life.

Nicole was present for her role with signage but truly went above and beyond to assist the HCC staff in ensuring the facility was prepared to shine. Moving furniture, calling in last-minute fixes and touch-ups really made the facility stand out as a jewel. Nicole was available to staff and really got her hands dirty at the last minute to ensure the facility looked at its best. By Nicole showing her teamwork and dedication, it highlighted to the other staff there that nothing is outside of our scope. It takes a team to get everything pulled together and it is not beneath any of us to get our hands dirty. Nicole did all of this with a smile and was always available for staff questions. A true shining star to the facility staff.

Nominee: Rigardo “Rig” Rush
Department: Executive/Administrative
Submitted by: Silvana Erbstein
I would like to nominate Rig for the invaluable help he provides us every single time something is requested. This particular time we needed Palm Cards “in a rush” due to a sudden change in phone number at one of our Health Care Centers in Miami-Dade. We asked Rig for help. He responded not only with the “perfect design” in English and Spanish but did so in just a couple of hours. His skills do not end with his marketing work; he also has a warm and cheerful attitude going always above and beyond in what he does. Thanks to Rig’s help, we were able to have the Palm Cards ready for distribution in less than 24 hours! Patients and the community alike received the updated information needed to receive the best possible care in a timely fashion.

I want to nominate Yadira for most improved employee of the year. Yadira has demonstrated noticeable improvement in her attitude and in her work performance. Her productivity has greatly improved and with her help, our paid not dispense reports have shown significant improvement (as highlighted in one of my managerial calls regarding the A/R). I’ve also noticed she interacts more kindly with her co-workers, which is important because we all need to feel comfortable performing in the workplace. I really want Yadira to know I value her as an employee and that I think she is an intelligent individual who owes it to herself to always give her best at everything. As a result, she may be surprised at how far she goes in life. I just want her to know I am very proud of her and I appreciate all of her efforts to improve for the betterment of the team.
Nominee: Raquel De Paz
Department: Pharmacy
Submitted by: La K
Raquel works hard every day to complete her job tasks in a timely manner and she rarely leaves behind work to be completed by others. Raquel has a good work ethic. I appreciate watching her set her own deadlines and personal goals for herself when it comes to completing her job tasks. She puts the pressure on herself to get her work done in a timely manner, which is a valued skill. For instance, when she has big homes to bill, she will approach me to inform me of her plans to get her work done, or to notify me of any issues she foresees as obstacles to completing her work for the day. This in turn allows me to make decisions accordingly to prevent problems for the team. Overall, I want Raquel to know I appreciate her as an employee and I appreciate the support she gives me as a new manager.
Nominee: Reina Hernandez

Reina works very diligently every day to ensure that she is providing the best patient care possible. Reina helps the team succeed on a daily basis. I’ve always appreciated her flexibility in job tasks. For example, she helps to minimize loss by helping to collect patient co-pays. She also helps minimize interruptions in patient care by coordinating her efforts with benefits counselors. She, too, helps conduct ADH calls when the ADH callers are preoccupied. There isn’t anything I haven’t seen Reina do. I’ve seen her do it all and I’ve been told she has done it all. I think she exemplifies a great team player and we appreciate all that she does for us on a daily basis. Just want to give her a warm shout out! THANK YOU, Reina.

Today August 26th, after a miscommunication with Temp Agency we found ourselves without a Pharmacist to open AHF Queens. All the high management was either on vacation or on the air flying out of New York, pretty much unreachable. It was 10 AM when I finally called Angie to ask for guidance. She was working at Chelsea at the time with another Pharmacist. In order to guarantee the well being of our clients and to safe the operations of Queen’s site Angie left one Pharmacist in Chelsea and she ran, literally, to open our location. Thank you to Angie’s determination and willingness to help we took care of all our duties. Without Angie we wouldn’t have opened. Thank you Angie. Thanks to Angie’s action we were able to provide our clients with their medications and scheduled deliveries were completed successfully.

I had an urgent delivery same day for a client’s antiretroviral. The delivery request from the patient came around 5 PM after all the drivers were gone for the day. Anthony offered himself to take the package to the patient even though this was not his direct responsibility. Anthony’s action embody what AHF is all about, putting our patient’s needs before our own. The patient was very satisfied for the quick response and above all the client avoided missing any dose of his antiretroviral and stayed adherent to his regimen.
Nominee: Negar Shirazpour

I had the pleasure of collaborating with Negar for a large scale community activation this past Sunday, called Church at the Park. As the lead point person for the Pharmacy team, I was pleasantly surprised to be met with her dedication and commitment to ensure that we had everything in place to be able to handle a large volume of participants for the free Glucose and blood pressure screenings at the Pharmacy booth. I appreciated her engagement with me and her team in the planning phases as well as all the way thru to the execution and clean up after a LONG & HOT day outdoors serving the community. It is always noteworthy when you see a person fully committed to the task and the cause. Negar, is a natural leader and I appreciate her help in making sure the flow of the day and our pharmacy booth was smooth sailing with all of the pharmacists, techs and students who were there helping out for the day. As someone who is always having to collaborate and work with many different people both internally and externally, I am privy to a lot of styles of getting things done. Negar is one who likes to make it happen. AHF has made a good choice in making her a leader of a team. She is great at what she does, and I hope to have more opportunities to work with her on community events!

We worked a large community activation called Church at the Park this past Sunday in South Los Angeles involving many different business lines and divisions of AHF. Samantha was a part of the pharmacy team providing the community with free Glucose and Blood pressure screenings. The day was hot. The crowds were full and the energy was high.

My biggest goal with every community event I participate in is to have a team that are all equally engaged and participating with purpose and passion. My experience with Samantha at this event and all others that we do, is to be commended. She helped from start to finish set up, execution and breakdown. It means a lot to me to have a colleague who demonstrates a work ethic and energy that matches my own. She exemplifies all that our mission stands for at AHF. Eric always has a great attitude and keeps a smile on his face. During a site visit I noticed him engaging with customers and clients, which brought a great feel to the atmosphere. Eric also thinks outside the box. Recently, he began working with OTC to help bump up testing numbers by providing store incentives for those being testing. This has worked well for him and is being continued. Eric’s personality brings great value to AHF as it shows compassion, hard-work and optimism. His presence is inviting and the initiative he has to accomplish his own goals within his position enhances all we are doing. More people are able to know their status as a result of his testing increase and he has been active in ensuring those of positive status are successfully linked to our clinic.

In such a busy clinic it was a breath of fresh air. I’ve never in years working here seen someone do it all at once. And with such timely professional manner. And with a smile on her face . She definitely did our “Above and beyond” . Just thought you may want to know. I’m going to send to Above and beyond as well. Thanks.
