Due to my flight time, I could not make it to a meeting in Midtown. She offered to meet the staff member and help me with the situation. She did this on her birthday. As a new part of the Southern Bureau, we are really encouraged to see the dedication and positive attitude presented by all. They gave Southern Bureau Leadership a great welcome and showed that Houston has the correct ethic and attitude. A true asset to the Southern Bureau.

Erin is an organized, thoughtful Nurse Practitioner. She is very professional and came into the Cleveland Healthcare Center with a great attitude that lifts up the moral of the staff in the office. She is able to maximize the one day a week that she is in the Cleveland Healthcare Center to see as many patients as possible and does it with a smile on her face. The biggest impact we have had from Erin being in the Cleveland Healthcare Center is that more of our clients are coming into their appointments and there has been a increase in new patients in the Cleveland Healthcare Center. She has been a joy to work with and all clients have left very happy after having appointments with her. Erin has also helped get patients onto PrEP immediately and yesterday did an entire intake for an unscheduled PrEP patient who was at high risk for HIV. She strongly believes that we can prevent HIV and will do anything in her power to help people at risk of HIV.

Dawn, you are a passionate, dedicated and driven only begins to describe you.
As the OA of Fort Myers Healthcare Center, you oversee a multidisciplinary activity but going above and beyond by helping out other HCC, like Oakland Park for the Annual Wellness visits.
Since your onboarding, you have dedicated yourself to creating a comprehensive approach to improving the quality of patient care and ensuring patient follow-up through the organization of these annual visits.

It’s the fast-paced, constantly changing environment and the variety of tasks that drew you to our HCC, but it’s your commitment to our patients that truly fuels your passion today. Not an easy mission everyday but you do not shy away from tough issues.

Your recent intervention has yielded promising results. The first, helping Oakland Park “make it over the hump”, reaching 53% completion. The second, and as a consequence of the first, is the improvement of the patients follow-up and reducing potential health degradation.

There is still a long way to go, but your discipline and rigor make you a true role model.

Thanks to your full dedication and thorough analysis, you have found an error in the Insurance Exception for the DOMO report that has been causing extended time for completing the task throughout the different Healthcare Centers.
By suggesting that if the report would automatically come up only for active clients and not ALL, prevents the OAs in having to add a filter to the report before they can view it. In fact, pulling the inactive patients into the results for the HCC is time consuming and irrelevant.
Your diligence has been a success factor in resolving the issue and bringing a welcome relief to her OA peers, saving precious time in a hectic environment.

Our Doctor Milton Garland is such a great provider in these 3 months that I have been working with him. I can tell how much love he has for our patients and How he really love our mission as organization. Almost 137 patients in this clinic are patients that transfer care to us due to Dr. Doctor is very knowledgeable about all the health system in Puerto Rico. Each of our patient express their love for him. He which that we can grow up as organizaton here in PR and has done as much as he can to make it happens. Thanks to Dr. Garland and his efforts and connections he get the contract with ADAP so we can start dispensing the ARV treatment for our patients here at our pharmacy making our clinic close to one stop clinic. It will make us get more patients and improve ours services. Dr Garland is always on the look to improve our clinic services.

Eugenia took over two duties that have really helped me out.

One, she helps us track down what clinic people go to if we need that information. There’s never been a single person before that we could go to and it helps me and Claudia out to have a starting point.

She also is handling all medical record requests for our Gilead litigation. This was a big burden on our department and it helps us out that we can refer our vendor directly to her.

She has also asked me for feedback in this matter and asked if there was anything that she could do better. I think she is doing a great job but I also appreciate that she wants to do a good job and was proactive about getting my opinion and constructive feedback. Eugenia is so pleasant to work with and I am glad we have her on the AHF team.

Related, I haven’t heard our records vendor complain about her work. I actually haven’t heard much about her work, which means everything is going smoothly. Thanks, Eugenia!

This is important because we are hoping to have as many clients as possible in our Gilead litigation and the sooner we get records, the quicker we can make a decision about the viability of their case.

There is also a diffusion of responsibility issue here at AHF, which frequently leads to more work and stress for me and others in the Legal department. It’s really nice and refreshing to see somebody be pro-active, helpful, and willing to help take something off of my plate.

Jim is always concerned about our clients and wants to ensure their needs are met. He recently planned, organized, and hosted a successful hurricane preparation event for the clients. He arranged for food, door prizes, hurricane information, and a “hurricane prep starter pack” for each attendee. There was a speaker and presentation given by Salvation Army. We had 16 clients and several staff members in attendance at this event. Jim did a great job of putting this event together. It was informational and fun. AHF strives to ensure our clients maintain their health and have a great client experience at our facilities. This event contributed to both, giving our clients practical information to ensure they are prepared for a hurricane and able to stay safe and healthy; as well as providing an outstanding client experience that demonstrates AHF’s holistic concern for our clients.

Although it’s hard to single out extraordinary workers in AHF’s Payroll department (since so many of them are extraordinary and efficient), I had to send an Above and Beyond nomination to Janelle. Janelle has been so receptive to my requests and needs for Payroll organization. I immediately feel the support and competency from Payroll when dealing with Janelle. I needed help with several employees ETIME and ADP timecards and Janelle provided prompt solutions to technical problems I experienced. Sandra’s participation in the HEDIS Project Sandra scheduled herself to assist in the participation of the HEDIS project. She demonstrated great flexibility, teamwork and loyalty to AHF. Sandra takes great initiative in her work as the QI Ryan White Project Manager. There have been almost 20 HCC’s added since Sandra started and she still has visited each HCC to train and she is only one person for the nation! Sandra has taken what she has learned from the Plans about HEDIS measures to the HCC’s to train how we try to eliminate duplication of effort and how one measure can meet several requirements. Sandra is one of the best in her job and deserves this recognition. I am so very happy she is on staff!

Joanne Tillman’s participation in the HEDIS Project while still doing her role as Health Educator. Beside facilitating the HEDIS measures incentive program, Joanne assist with abstraction overreads for HEDIS. HEDIS Project and Risk Management for Ca and Fl Tiffany has had to juggle Risk Management for both Ca and Fl while participating in the HEDIS Project. Tiffany has demonstrated a great flexibility while still maintaining the highest of standards within the confines of resources. Tiffany is working on the CAPS process, data stratification and added standards for grievances while working on all the other QI operations required for FL and Ca. Tiffany has a great common sense approach to Risk Management and is a subject matter expert. I am very happy we have someone with such expertise to triage our grievances. She has keen sense of when an issue needs escalation. I am so happy she is on the QI Team.

Emelyne has been highly successful in assisting QI in medical record retrieval for the HEDIS project.
The medical records contribute to the increase in revenue received from the data utilized from these records to get positive hits for each of the measures. Emelyne also processes all the grievance at the same time and is always timely. Grievances have increased so Emelyne has had to work extensive hours without additional pay since she is hourly.
Emelyne has gone “above and beyond” to assist in making sure AHF has the best results possible.
She is an A+ employee. I am so grateful to her and her “can-do” attitude. She is the very best we have.

Lori Bradford has been very helpful and patient with me. I began as a patient with AHF in March at that time I was experiencing life challenges in many different areas. Lori took the time and professionalism to direct me and assist me to the best of her ability. She follow HIPPAA guidelines to the max. She has boosted my spirits and helped me get on and stay on track. Even checked on me and my well being as needed. Also factored in patient teaching with importance of safe sex and proper medication regimen. I appreciate Lori. She is one of the reasons I have decided to continue as a patient with AHF. I am a nurse as well working with Mrs. Bradford I must say she is a great nurse case manager. She doesn’t complain always have a smile and make me smile even when I was very down. Mrs Sherry Nolan was very helpful. There where services we need that we didn’t have Mrs Nolon arraigned for us to be connected with several services. Such as services for produces like pull ups and bed pads. Also she arranged for us services for intake for nursing services. Mrs Nolon always called and checked to see how things where going, if we were making our appointments and to see how we were feeling whether we were doing better or worse and offered suggestions to see the Dr or go to the hospital. Mrs Nolon’s genuine concern was a much need help. It help move us forward. I thank Mrs Sherry Nolon for her expertise in what she does. Vanessa Bender mother of Brian Bender died 8/27/18 The services Mrs Nolon connected us to helped make things easier to maneuver. She arranged rides for Dr’s and hospital visits as well.

During Gardena transition, all stores had to fill medications in stores and this caused a change in workflow. Angela worked overtime and helped ensure patients get their medications on time. Angela played an essential role in the transition by coming to work early and staying over until the job was done, Her dedication and hard work is appreciated and is a great example of a team player. Her actions has helped may patients get their medication on time and for the pharmacy operations to run smoothly.

Natalie is extremely hard-working and accomplishes a huge amount of work each day even though she is doing the work of 2 people due to an unfilled position at our location. She is meticulous and detail oriented and if I have any questions about any matter pertaining to processing, billing, or procedure I know that I will get a correct answer from her or she will know exactly who to contact if she is unsure. Because of her actions and availability to help, I am able to accomplish my job more easily and get meds out to the patient in the timeliest manner possible. Because of her processing knowledge, if there is an insurance issue, we can all go to her and either get the computer codes to input into the correct fields or she will know who to call to have it taken care of on the processor’s end.

Bertina always provides superior customer service; however, yesterday, she went above and beyond for a new patient. She was on the phone for over an hour being sent to 4 different phone numbers to try to get a patient’s Biktarvy covered through his insurance. She kept the patient updated on her progress while he was waiting in the waiting room and on hold with the insurance over the phone – patient was very pleasant the entire time and appreciated how much she was devoting her time to figure out his insurance. Finally, the insurance person she ended up resolving the issue with stated, “don’t hate me, I’ve been putting in the wrong ID number” that she had given him multiple times. She never lost her patience , was professional and sympathetic to the patient who had been waiting. Patient appreciated her perseverance and professionalism. Every patient deserves to be treated as if they are the VIP of any business. She not only made us look good, but continues to make the AHF look good as well.

On 07/11/19 @ approximately 4pm Nadine was leaving a delivery and was flagged down by a pedestrian and asked to call 911, Nadine noticed a unresponsive male being pulled from a vehicle parked on the side of the road. Upon further inspection the male was found to be unresponsive. Nadine called 911 and stated chest compressions. Another gentleman on scene took over but stopped when he heard the ambulance in the distance. Nadine resumes chest compressions until the paramedics were on scene and then the subject took a breath and started breathing on his own Nadine was able to literally bring someone back to life. Caring for people is what AHF is all about this is a prime example of helping someone who at that moment could not help themselves.

Sonu Patel goes above and beyond in helping her patient receive their prescriptions. Sonu Patel helped one of our patient from wood hall with her pills box. Patient mixed different pills and added more pills to her pill box. As a result, even doctor and patient were confused types of medication in pill box. Sonu Patel received the call from wood hall doctor and helped doctor arrange her pills. She also spoke to the patient in the importance of not mixing the tablets. As a result, patient and doctor were both satisfied and were very happy to receive help from our knowledge pharmacist Sonu Patel.

PHD Houston wellness had to move from the Binz location to the Hermann location. Diandra took charge and moved items and her staff to the new location and was up and running to ensure client needs were met. Being down 3 staff members, Diandra worked tirelessly with the MTU team to ensure that the wellness can still function. From answering the phones to ensuring the clients get their questions answered, Diandra ensured there was a steady flow during the wellness hours Even though we moved from one location to the other, Diandra ensured that the clients needs where met and didn’t allow the move to affect wellness services. As one of AHF’s goals are to be patient centered, Diandra made sure that this was priority.

I asked Angelica to travel to Houston with me to assist with the move to the new location. Angelica took charge and was able to assess the needs of the new wellness clinic, ordered all that was needed and will be going back to ensure that they have all items set up. As a new manager, she continues to show great leadership skills and is proactive in resolving issues and matters that arise. Also she continues to be a great team player and is solutions driven. Setting up and moving could be stressful some times but when you have team members who are willing to step in and step up to ensure a smoother process makes a lot of difference, For Angelica to quickly assess the needs of the wellness clinic to ensure that they have the tools and capacity to serve clients is commendable.

The PHD Operations team is consistently there to help PHD programs whenever in need. Countless number of times Ops is requested to help out with a task last minute and the Ops team is always there to see it follow through. Many times, testing staff is not available to run an errand or work on other programmatic needs, however, Ops is the team that you can count on to make those help make tasks and projects come to fruition. From fulfilling orders, dropping off testing supplies or taking MTUs for service, you know Ops will be there to make things happen. I couldn’t ask for a more supportive team than Ops. Ops goes out of their way to ensure that testing services go uninterrupted and testing events are set up for success. Much of the success that testing programs have is due to the direct assistance received from Ops. They are an invaluable part of the Public Health Division.

Quincy has reached out to the most difficult to reach people and brought some into care. Beyond that he continues to reach out to these people and keep them in care.
Beyond that he is always thinking of new ways to engage people in care, beyond using the mobile testing unit almost exclusively at night to reach more difficult to reach people.
Recently, Quincy has become our back up phlebotomist, coming in even at times he is not scheduled to be working to fulfill that need when we lack an MA. He does it with the joy he has at all times.
Without Quincy fulfilling our need for phlebotomy when our MA is not here we would and have lost patients, or not been able to care for them as AHF and I would like to.

Beyond that, in his capacity with the MTU, he presents a wonderful example of a kind, friendly person who relates to all that come to be tested.

We consider ourselves very fortunate to have Kelli be the Pharmacy Sales Rep of the West Hollywood testing team. Kelli brings with her a captive energy and innovative ideas that has helped the West Hollywood Fixed Site testing team reach levels of success unseen before. Without Kelli’s diligence, hard work, and infectious energy, we would not have been able to meet our goals. Whether it is an early morning strategy meeting, an entire weekend’s worth of activities, an event that falls on her birthday, or even stepping in to cover another Sales Rep, Kelli can always be counted on to be there with a smile. She goes above and beyond to ensure that the testing teams have all the support that we need from her and more.
