He tirelessly reaches out to those at risk for falling out of care and encourages them to attend their appointments and be adherent to their plan of care, thereby reducing the community viral load and progressing us toward our goal of ending the HIV epidemic. This past month, he completed 289 calls, 113 texts, 18 F2F visits, 67 successful contacts and identified/assisted 6 retention patients who had moved (180-day report) with transfer to another AHF HCC.
From this 180-day report, 27 patients attended their first retention apt and 32 attended their second one during the month of February.
Even more striking to me than the numbers, though, are the stories behind some of these patients. Please see a couple of Lewis’ success stories:

Outstanding performance for a patient in need of a respiratory medical device. Ms. Judy McKnight, Registered Medical Assistant/ Referral Coordinator at HCC North Miami Beach, upon recognizing a problem immediately assessed the situation with a non-participating PHP health plan vendor, which there was no follow up on vendor’s behalf. Ms. McKnight promptly contacted three different participating vendors, collected all necessary documents and was able to successfully obtain the needed equipment for patient/ PHP member in a timely manner. This situation, if not handled swiftly, may have caused an emergency room visit or hospital stay if not for her analytical thinking and quick response. Nominee’s act of recognizing a high-priority situation and her prompt attention-to-detail improved the access to care for a PHP health plan member. Her performance is in accordance with the high standards and traditions of AHF public and global healthcare service.
