Nominee: Christina Salmon

Department: DOM

Submitted by: Kristen Harrington

The Delray Beach HCC has had a difficult time with neighboring tenants fear of sharing the buildings public restrooms with our patients. Today, and elderly patient struggling in the bathroom, did not make it in time resulting in a mess on the restroom floor.  this was brought to the attention of HCC staff and Christina Salmon, the on-site Case Manager, cleaned and sanitized the buildings public restroom floor. This unpleasant task that Christina took on, helped get ahead of an impending complaint by the landlord. By the time the landlord reached out to complain, the floor had been cleaned and the issue diffused. Although this does not directly impact patients, it aided in maintaining a clean sanitary space for our patients and staff.  Additionally, because we have experienced issues with other tenants displeased with having our patients share the restrooms, her actions resolve an issue quickly, effectively, and help us maintain a responsible and friendly business relationship with our landlord.

I received a recent email with very positive feedback reinforcing how valuable of an asset Kepler Verduga is as the Office Administrator at AHF Oakland Park HCC. His leadership style and positive attitude are a winning combination. Kepler is changing in a good way the culture of the OKP HCC. AHF is lucky to have the gentleman on our team. Good morning Dan,

As per our conversation this morning. We can follow up next week. I will be here on Monday. Tuesday & Wednesday, Thursday I am in Safety Harbor interviewing 2 candidates for the Ryan White/IMM RN Case Manager position and showing Patrick Saint Fleur the AHF facilities in Safety Harbor and St. Petersburg. I will be back in the office Friday.

I was at Oakland Park yesterday for an interview for a Ryan White nurse. Karen Haughey has asked me to provide Nursing support for Patrick’s Ryan White Nurses. Also, I worked at Oakland Park for two years as one of the PHP/PHC RN Case Managers.

I want to say how impressed I am with Oakland’s team, the Health Care Center as well as the pharmacy. I want to single out Kepler especially for providing such great leadership. There is definitely a change in the atmosphere there.

Congratulations on the training and leadership you have provided. Your work is definitely a concrete response to the email we all got from Michael Weinstein.

Nominee: Kenya Wade

Department: DOM

Submitted by: Daniel Sheridan

I recently received a beautiful email from a patient at the AHF Piedmont HCC where Kenya is the Office Administrator. It is a testament of how Kenya treats our patients. Dear Mr. Sheridan,

I write to you to convey my heartfelt gratitude to the staff of AHF Atlanta, and in particular, Ms. Kenya Wade.

I recently visited the center to get tests done and from the very first go, with Porsha and Amber, I was made to feel at ease.

However, as good as they were it, is the care and help of Miss Kenya Wade that has struck me the most. Miss Wade took time to walk me through areas of the process, even on behalf of another staff who was not there at the time: counseled me and: has been there to help via the phone and to make necessary liaison on my behalf.

Not only has Miss Wade done her job: she has gone the extra mile and has displayed such humanity while doing so.

Thank you sir for ensuring that your staff is not only competent, but is demonstrative of the attitude and care for making the lived experience of patients, clients and stakeholders of AHF a positive one!

With gratitude,

Nominee: Josie Garcia

Department: DOM

Submitted by: Jennifer Gjurashaj

I work with Josie Garcia, Front Office Clerk/BC at San Diego HCC, to manage Ryan White funding for the clinic. The clinic has experienced nursing vacancies recently, and the positive attitude Josie exhibits towards every patient goes a very long way in making sure that patient care is not impacted as a result of the vacancies. I got to see this during my last visit to the clinic. She has a strong team-player mindset, helping the back office operations on top of her duties to keep the flow of the clinic running smoothly. She also maintains a sense of humor with the other staff and the patients, which I witnessed to be a constructive way to decrease the pressure of certain situations and create a positive atmosphere. She connects with the patients to make them feel at ease the moment they walk through the clinic door. There’s no doubt that this type of atmosphere plays a part in contributing to retention.
Nominee: Giovan Hernandez

Department: DOM

Submitted by: Jennifer Gjurashaj

I work with Giovan, the OA at San Diego HCC, to manage the Ryan White funding at the clinic. I witnessed his strong leadership skills during my last visit to the clinic and just had to write this nomination. On top of his duties as OA, Giovan has been picking up some of the administrative duties of nursing in the absence of an RN Nurse Manager (vacant position): routing calls from the nurse’s line to his attention, working with patients to ensure specialty care referrals are processed and that they have access to medications according to their needs, working with his team members to fit in patients for vaccines at the last minute, and handling pressure quite graciously. All of this while finding time to accommodate audit questions and general performance questions for the Ryan White grant. He is definitely going above and beyond! His attitude and willingness to help the patients, no matter what the circumstances, is a true demonstration of the mission and purpose of AHF. The stress of being pulled in many directions at once is not easy to grapple with, yet how Giovan handled this pressure was a tell-tale sign of his patient-centered approach to his work.

With or without this award I just want you guys to know that she saved my life and for that I thank you.

Jocelin is an exemplary employee, and such a joy to work with.  Her positive attitude, optimism, and smile makes for an enjoyable work space. Jocelin exhibits above and beyond behavior each and every day. While Jocelin displays many impressive traits, her strongest is her problem-solving skills. A few examples are:

1. Medication access: Jocelin does not end her work day until she knows all of her patients are connected with medication. She routes buses, calls other AHF locations, uses multiple forms of communication, coordinates with drivers, and works tirelessly solving everyday issues that most people would table until the next day.

2. Working in a multidisciplinary team: Feedback clinic pharmacies have received is there sometimes is a disconnect between transferring patients back to the clinic for services that are never received. Jocelin physically walks over to the clinic to ensure she is connecting patients with a BC, social worker, RN, or front desk staff so patients are not forgotten or lost in the system. Not only does she appropriately identify who can best help the patient, she follows up with AHF staff (and the patient when necessary) to ensure needs were met. This is not required of her, but her compassion leads her to always guarantee patients are receiving the best care possible.

Nominee: LaChay Dixon

Department: PharmacySubmitted by: Michel McVicker-Weaver

LaChay has taken it upon herself to package condoms and palm cards into neat little kits and keeps them stocked in the lobby. It’s a great way to send home info about our services in a ready to use kit that is attractive. This demonstrates her commitment to the mission and AHF culture.
Nominee: Pamela Woods

Department: PharmacySubmitted by: Tracy Gum

Whenever I see Pam, she is always smiling. She greets everyone she meets daily and is a pleasure to talk to. Today, my patient described her in the same way. He states she makes his day when she delivers his medications to him. He added, “She also brings my dog a dog bone each time she comes!”

This is something she does not have to do, but certainly goes above and beyond and makes our customers/patients happy.

Nominee: Bobbi Crouch

Department: PharmacySubmitted by: Tracy Gum

Bobbi, our pharmacist, assists our patients everyday. One of our patients is a newly diagnosed diabetic. After being discharged from the hospital with he new diagnosis, he went to our pharmacy to pick up his new prescriptions. He relayed to me that Bobbi took him into a separate room and demonstrated how to use his new glucose meter as well as how and where to inject his insulin. (He had not been shown in the hospital). Bobbi goes above an beyond daily, but taking the time to help our newly diagnosed diabetic patient how to use his diabetes supplies helped give this patient a sense of relief.
Nominee: Kylee Burnthorne

Department: PharmacySubmitted by: Tracy Gum, NP

Kylee always goes above and beyond, whether it is assisting the provider, office staff, or patients. Most recently, a patient was able to fill his prescriptions with AHF however due to insurance restrictions, he was required to use another specialty pharmacy/mail order. The patient came to pick up his medications from AHF when this detail was discovered. Kylee called the specialty pharmacy, transferred the prescriptions and arranged for overnight delivery of his medications and averted the patient being without his medications. Kylee is an exemplary employee demonstrating AHF’s goals. Kylee impacted both the patient and provider by taking the time to ensure the patient had his medications, and that transfer of pharmacy was completed.
