Important Information For Molina Medicaid Patients
Dear Patient,
There have been many changes to Medicaid managed care plans in Florida in 2019. Some of these changes affect AHF Healthcare Center and you!
Please be aware that effective July 1, 2019, AHF Healthcare Center will NOT be part of Molina Healthcare’s provider network.
If you want to continue to see your medical provider and staff at your AHF Healthcare Center without interruption, you must enroll in one of these Medicaid managed care plans. AHF Healthcare Centers in Miami-Dade County accept:
- Staywell (Note that University of Miami Medical Group (UMMG) is not part of Staywell’s network for adult care.)
- United Healthcare
- Sunshine Health
- Prestige
If you would like to change your Medicaid plan so you can keep using your AHF Healthcare Center without interruption, call the Agency for Healthcare Administration’s (AHCA) Choice Counseling line. Call (877) 711-3662, Monday through Thursday 8:00 am to 8:00 pm, and Friday 8:00 am to 7:00 pm. TTY users call (866) 467-4970.
You can also change plans online by going to www.flmedicaidmanagedcare.com and logging into the member portal. If you have not used the portal before, you will need to set up an account. You will three of the four items below to set up your account:
- Your Medicaid member number (found on your gold Medicaid ID card)
- Your Medicaid ID number
- Your social security number
- Your date of birth
If you would like to voice a complaint to AHCA about any problems changing your Medicaid plan, please call AHCA’s Medicaid Helpline at (877) 254-1055, Monday through Friday, 8:00 am to 5:00 pm.
