Our “Above and Beyond” employee, Reginald Shiloh, is a behind the scenes, “HIDDEN GEM” we would like to recognize for his dedication, loyalty, and remarkable work with the JAX PHD team. This employee has been faithful to AHF for a year, as an Outreach Specialist. He is the longest standing Outreach employee we have had on the team this far. His passion and compassion to raise awareness about HIV/AIDS is extremely noticeable and appreciated by many individuals. He takes the front line of prevention very serious, when on duty he engages, empowers, an educates the community on HIV/AIDS. His eye and ear for recognizing those who don’t know their HIV status and those individuals that have been lost-to-care is very impressive. He was able to encourage a young man that shared his HIV status with him to get back into care, when the young man told him, “he would rather die.” AMAZING!!! When the wait time for HIV testing can be a little lengthy, he has this way of redirecting peoples attention off the wait with educati Listed in notation above. As a result of this employees dedication and passion, the number of individuals that now know their HIV status has increased tremendously. This behavior is a result of the impact of High impact Prevention. Since I started at Lithonia HCC, I have seen Antelita grow and become more and more flexible to the business needs of our HCC and, also, the needs of other HCC’s. Antelita has worked at other HCC’s providing training to staff more and more over the last year. Most recently, she assisted me in training a new nurse for AID Atlanta. Without complaint, she made the nurse feel welcomed and supported. She also assisted me in moving the nurse forward in her understanding of our culture and philosophy. Antelita is a true team player. By setting the high standards that she does, not only for herself, but for the staff that she assists, I am hopeful that we have better trained staff and a more effective workforce in our area.

From an email sent to me by Dr. Sandra Win …….Although Daniel’s contract is in a different service line, he has gone above and beyond to help our clinical team to deliver quality patient care.
As his schedule allows, Daniel has helped with specialty referrals, radiology referrals, linkage to psychiatric care, supporting our one and only medical assistant, and, generally, assisting me
as the RN in our HCC. He is a wonderful team member, caring health professional, excellent RW nurse manager, and I’m fortunate to have him on our team.
Daniel’s ability to see the priorities and jump in to make it work showcases our core value of being nimble. Our mission describes “cutting edge medicine and advocacy….” Daniel advocates for our RW patients on a daily basis, he expanded the mission to what was needed during a time when the HCC needed help.

Roxan came to AHF in March – midstream and head on into a re-audit of the PHP Medicare Plan. Without hesitation she took up the leadership of the team, the concerns, the training, the re-training, the process and everything else that went along with this very intense review of work processes. Her leadership and attitude brought this team to a successful finish. Her poise and professionalism during the intense portions of the review with CMS showed her expertise and dedication to AHF. Roxand dedication to PHP and to AHF showcases our Management Philosophy – she is MISSION DRIVEN – giving her all to this audit, LEARNING and HANDS ON – diving deep into the work process, she was able to understand and show the auditors how this is done. ACCOUNTABLE – she owns what she does, TENACITY – her “can do” attitude and we WILL do this – paid off. We could and we did on Roxan’s watch.

Christopher has gone above and beyond in the relocation of the K Street HCC. He has been so organized and on top of the entire move. Months later, we ordered artwork for the location and Christopher had to coordinate with the building and vendor to have the artwork delivered. There was a huge obstacle in ordering because they didn’t fit in the elevator and there was a lot of back and forth with the vendor. Christopher stayed on top of everything which ended up being a whole 4 week long process. Christopher showed his patience, communication, and organization skills through this project. It took a lot of communication with our vendors to have them delivered but Christopher was able to problem solve and even find a contractor to help with the install. He stayed true to AHF’s mission and was very nimble.

It is on a very frequent basis that we get clients at our Chicago HCCs who need a password reset for their AHF patient portal account. Clients usually report that they “lost” the initial letter or they state that they were not registered for the portal only to find that their email was already in use which means they were previously registered and attempted login.As soon as a request is sent there is usually a response from Ama saying “responding” or asking for additional information if needed. The clients report that whenever they call the HCC regarding any login issues with the portal and/or inform staff at an appointment they usually get an email with password reset information on the same day. A big thank you to Ama!

Jaypee spent over 1.5 hours processing and getting an override on prescriptions for patient in need Patient and her family were displaced by the Wild Fires in Sonoma county. They forgot to grab medications. They are put up in a hotel here in San Francisco. We received a call from Patient’s Mom practically in tears. Due to Jaypee’s efforts, we were able to process medication and fill in MediSets and get to patient that afternoon. This cuts directly to our mission statement “Cutting edge medicine and advocacy”

Being diagnosed as HIV-positive has been a difficult process. Lori Bradford, my Medical Case Manager at AHF was instrumental in helping me make the emotional adjustment with myself and even how to view myself. I think do her words daily as my make dietary choices and exercise 5 days a week. She has encouraged me to talk about this either with family, friends and or in group therapy. 6 months after my diagnosis, I’m encouraged that I can have a full life. Lori is always ready to listen. Excellence is a common thing done uncommonly well and Lori is an excellent advocate for this community. Thank you, Lori! Lori is resourceful, always ready to find a way to get someone (me) insured, or enrolled in Medicaid so that we can get therapy, or connected with a therapist, or to begin a fitness regimen. Her resourcefulness is a godsend for someone with needs support in a vulnerable time. I’m certain that without AHF and with staff members like Lori, I wouldn’t be alive and as well-adjusted as I’m daily becoming.

Patrick has been an intricate part of the development of the California Regional Group, A statewide Quality Improvement group that is led by myself, as part a national HRSA collaborative. One of the duties that I have a the leader of this group, is to plan and facilitate Ryan White consumer and Provider trainings throughout the region. I can proudly say that, with Patrick’s assistance, we we able to facilitate two amazing Consumer/Provider Quality Improvement Training Summits this year. Patrick made himself available to help plan and support these events. Patrick not only makes sure that we have the resources available to host community trainings and meetings, but he also provides an immeasurable amount of support to our AHF support group in Upland, CA. I can call on Patrick to support me in educating clients regarding medications, prescription assistance, or Prep/Pep. Although he lives hours away, in San Diego, Patrick is a present support in our clinics in the Inland Empire and we appreciate his willingness to help out whenever asked.

I observed that Winter an her team at Healthy Housing are very responsive and proactive with processing referrals to Healthy Housing and eliminating barriers to getting our clients housed. I have observed that the Healthy Housing team is dedicated to eliminating homelessness by gently and quickly navigating client through the application process and connecting them to resources that will make the move-in experience as smooth and less costly as possible for the client. The Healthy Housing team is well connected to other RW service providers in the community, which helps in the facilitation of multi-agency collaboration. It is a pleasure to work with Winter and the team there at Healthy Housing and I am honored to say that with their help, we are able to make a difference in the lives of our homeless clients, one client at a time. Winter and her team were able to facilitate the move in of a mother and her three children into Healthy Housing in less than 5 days. Winter coached me on how to communicate with a shelter manager in order to prevent my client from being on the streets while she and her team worked over the weekend to get my client approved for move-in to Healthy Housing. Staff Dominique was able to wait for my client to arrive at the property and facilitate her move in after hours, on a Tuesday night. Because the client was able to get housing, she is now linked to AHF for HIV treatment and has reengaged in medical care! Her children are now stable and enrolled in school and she is on her way to rebuilding the life that she desires for her family.

Maria’s productivity and accuracy in our new Centralized Billing rollout has been impressive. She has been willing to go the extra mile to make sure prescriptions are dispensed efficiently and accurately. Maria has been paying close attention to detail such as synchronizing medication for 30 day durations, and checking days supply. This makes sure we don’t set ourselves up for audits, while also providing excellent patient care. Maria has been responding quickly to emails and takes initiative. I started working for AHF Washington DC. Since December of 2014. I was working as Agency pharmacist where there was no pharmacy manager there was one per-diem pharmacist Al-Sean I continued working for about 4 months until AHF find a pharmacy manager. The pharmacy was disorganized and the Pharmacy techs were from temp agency. Joann started working in March 2015. She became a pharmacy manager. There were a lot of challenges she has to face including untrained tech, a lot of unhappy customers in general staff issues. During 2015 and 2016 I continued working for AHF through Temp agency called GLC and later new agency called Career Rx. I became AHF employee in March 2017 and start working regularly with Joann. I am testament for Joann hard work to turned around the pharmacy business and be able to manage 3 clinics patient in addition to several outside clinic patient. The entire team has been demonstrating great spirit and motivation, each member being complementary.
Together they strive to reach key indicators on daily routines but also additional imperatives such as audits.
As a result of this above and beyond attitude, the team has achieved great performances :

– The HCC has some of the highest revenue per visit in the Southern Bureau

Due to a power outage, many medications were outside temp recommendations. Alexander compiled a list of recomendations The document was then shared widely – the research to call all the MFR’s would have taken a long time to make – and significant effort was put into developing the document. Further it was shared widely so that all pharmacists had the current info incase of similar experience – VERY GOOD information to have immediately on hand.

I would like to recognize the three staff that went above and beyond to support in replacing my stolen laptop.
My work laptop was stolen while in business travel in Oakland and I notified HR and IT of the incident.

Once I put in the ITAR the Mohandoss personally reached out to me and assisted in expediting the request, He did this knowing it was my only source of work equipment and I needed it to continue daily operations.
He and his team of Michael and Bernard were on it to process the request, expediate it and deliver it to my office as soon as I returned from the trip.

In going through my property being stolen, it was nice to get such immediate support and replacement of my laptop.
I truly appreciate it!

IT’s support enabled me to continue to get my work done without much challenges. Their support brought back a sense of normality in a chaotic situation.

She made a profile on line for me to get my income paperwork. She reminds me to tell the doctor about my symptoms. And she has a company delivering food to my house. She is an all around hustler for her patients, lemme tell you! I don’t know what the mission of AHF is. I just know that Lori is trying to help me get housing. She found a dermatologist for me. Whatever I need, I know I can call on her to try to get it for me. She is THE BEST!

Dr. Deborah Davis is the most caring, kind dentist I have ever had. She and her staff Michelle, Carlos and Edith are incredible and care for their patients like they are family. They help put me at ease in a difficult situation. They always make you feel like you are the most important patient to them. I can thank them all enough for their kind, compassionate care. Dr. Deborah Davis and Staff offer dental care Above & Beyond. The kind loving care that they give in this office is amazing. They treat yo as person that matters and do everything possible to support and care for you. I lost my mother and had an appointment with Dr. Davis and I was seating in her office and she showed such love and care and understanding as she hugged me in her office. I would like to thank Kelli for coordinating the MTU and pharmacy departments. 51 tests were completed on that day. All AHF staff present has an opportunity to speak to the Ugandan community members. AHF passed a long several fliers and brochures about AHF health centers and health plan. Kelli spent a lot of time coordination with all AHF departments involved. Kelli not only provided health education but was very engaged with the community and answered question asked individually. She was very pleasant to work with and made a tedious process so easy to go through. The community members were pleased with Kelli and the team and can’t wait for the next event.

Brian is always one to help and listen, even when it’s personal. He has a long-time client who was recently in the building and the client had a bathroom incident and Brian ran out to get the client new pants so he could get home. In addition to his caseload, Brian also works in the HCC to make sure the social service needs of the patients are met, as well as running the Men’s Support Group we host here. Brian has been leading Men’s Group since before I started with the agency 9.5 years ago, so talk about dedication. He attends every event and is always, always willing to help in any capacity needed. Brian’s care for his clients is truly above and beyond and is always willing to do whatever it takes.

I believe that Rebecca Strong is one of the strongest leaders working on the Advocacy Team. She is kind but direct and is always willing to step in at a moments notice to help out. She is the essence of a “Team Player” and largely supports the work of the Mobilization Team.

Rebecca works evening, weekends and holidays to make sure the team has what is needs to function. She is tireless in her work for AHF’s mission and she never says no.
