Advocacy & Mobilizers
Radical Cooperation: We will host 5 Twitter Rallies each focusing on different service lines of AHF

Affiliates North & South
RX Growth: get as many affiliate patients as possible to fill a prescription in an AHF pharmacy during the Olympics (7/15-8/15). Affiliates will be responsible for tracking patient names and DOB as well as the employee responsible for the referral, and will provide listing to Finance to validate that the patient actually filled the RX with AHF. All prescriptions will count regardless of whether the prescription is for primary care, HIV+, or Prep. Affiliates will be tasked to be as creative as possible to drive traffic to our pharmacies and promote our services. The goal will be for our pharmacies to continue to provide the excellent quality service that they already provide and hopefully retain or convert as many affiliates patients as possible to stay with us going forward.
Radical Cooperation: Affiliates to participate in some kind of activity or event on behalf/alongside AHF during the Olympics (7/15-8/15). This could be attending an AHF protest together, hosting a joint testing event, participating in twitter rallies etc. Teams can be creative with the goal of working together for a purpose, and not in silos. At the end of the Olympics, teams will submit all of the joint activities they hosted/participated in along with employee names for consideration.

Affiliates West: WORLD
Radical Cooperation: Inform WORLD clients about the availability of Wellness for PrEP and STI testing and about AHF pharmacy for PrEP fills and encourage them to consider these services as appropriate and host at least one or more events with AHF to bring awareness to the work in Oakland of both agencies.

AHF Pharmacy
1. Conversion of Truvada scripts to Descovy
 a. Goal: complete excel spreadsheet on K drive which lists all active Truvada patients. Mark action taken along with outcome for trying to convert to Descovy
 b. Metric: pharmacy judged on completion and successful conversion to Descovy based on answers on spreadsheet
2. Re-capturing 35-Day report patients
 a. Goal: by focusing more effort on 35-Day report and other established resources, pharmacies can bring patients back into care before being identified as a pharmacy drop, thus improving pharmacy retention.
 b. Metric: new report to identify % of patients on the 35-day report that have an additional refill activity in the pharmacy (ARV only), excluding 90-day supply patients between day 60 and 119.
3. Forced Mail Order Opt Out project
 a. Goal: increase pharmacy participation in the Pharmacy Sales team project to guide patients in the Opt Out process of forced mail pharmacy restrictions from their insurance plans
 b. Metric: pharmacy judged on participation in Salesforce in which patients are identified and handed off to Pharmacy Sales team to assist in the Opt Out project. This is the established method that was trained across the country earlier this year.

Finance
1. Finance Directory Glossary by Department, by Function with contact/information (person(s)/email):
 Objective: Provide people in AHF with a comprehensive Finance Directory, inclusive of key areas of responsibility so that others in the organization know who to reach out to (or email address)
2. Finance Training Series could be called “Everything You Wanted to Know About Finance But Were Too Afraid To Ask
 Objective: Other Departments within Finance and Outside to learn more about different areas of Finance on key topics and/or training. Opportunity for them to ask questions.
3. Finance Department Survey to Key Business partners
 Objective: Get 360 Finance feedback of what we are doing well and what we could be doing better

Healthcare Centers and DOM
DOM will be using the percentage of improvement of our patient Non-Complete Percentage metric.

Healthy Housing Foundation
1. Complete 5 move ins per week.
2. Keep background check turnaround time within 7-14 days.
3. Maintain occupancy levels above 95%.
4. Increase rent collections by 20%.
5. Create resident satisfaction survey with 75% satisfaction goal.

Housing is a Human Right
2,000 signatures on a petition against criminalizing the homeless.

Human Resources
1. 95% completion for AHF University May trainings
2. Confirm all supervisors/managers are correct in SuccessFactors and have all been enrolled in Leadership Training
3. Create DE&I scorecards with recommendations for each business line

Information Technology
1. Develop user friendly custom reports that can help to track our Patient Data and their survey.  We will also include secret shopper reports.
2. Implement system improvements to help our Staff’s and Patients ( New Patient Portal, Cerner to the Cloud, Call Tree for HCC, and Custom forms for HCC).
3. Optimize the Service Now with user friendly forms

Legal

Managed Care
Radical Cooperation: Improve and streamline the HCC/MC referrals process. 
Managed Care: 
Goal 1: Authorization turnaround time 5 business days or less, standard is 14 days. 
Goal 2: Publish a list of preferred specialist for the Direct Referral process 
Goal 3: Publish a list of preferred specialist for referrals (authorizations) 
Department of Medicine: 
Goal 2: Evaluate current referral status and process for all insurance LOB 
Goal 3: Establish TAT for referrals 
Goal 4: Identify gaps in training 
Goal 5: Improve turn around average by 20%

Marketing
Radical Cooperation:
The marketing department will work with sales, regionals, mobilizers affiliates and affinity groups to create a national events calendar. The calendar will contain events, sponsorships, engagements, etc. and will encourage local AHF staff participation.

Out of the Closet
• Create and implement an event or strategy that will increase census and/or retention – Event can be done on their own or through radical cooperation with other AHF service lines.

Public Health
Rx Captures: We will create a system in which we will send leads to AHF Pharmacy.
