AHF Summer Retention Contest (RetCon)

For the month of September, we’re running a competition to ramp up retention. The HCC team with the highest met goals will receive big cash awards for their success.

 Goals:

  1. Most improvement in non-completed visits = 35% or below at the end of Sept (DOM/HCC Staff).
  2. Most patients not seen 6-12 months, completes a visit in Sept (CHW and DOM/HCC staff).
  3. Most patients not seen greater then 12 month, completes a visit in Sept (Sales and DOM/HCC staff).

Leverage your entire HCC team of community health workers, sales reps, and DOM/HCC staff for the best results. HCCs will compete against sites with similar census size. Three tiers: small, medium, and large. Three sites in each group with the biggest improvement in all three categories wins.

That's a total of 9 HCC teams that could potentially win!

Tips & Tricks:

  1.  Work the 104 day report 
  2. On Sept. 1st, pull your unscheduled reports for 6-13 months and 13+ months. Then, divide up the lists among staff (HCC staff, Sales reps, CHWs).
  3. Start with trying to fill unused slots in the month of Sept.
  4. If you have high-no-show patients scheduled for in-person appointments, convert them to telehealth to help drive completion.
  5. Use the new non-complete scripting to let patients know it is important for them and the staff to keep appointments.
  6. Discuss your daily plan in every huddle!
