If you are receiving this message you are eligible to receive the COVID vaccine including the 2nd booster (4th vaccine). The booster vaccine we are currently providing is the Moderna Vaccine. 

Here are three ways to get it:

  1. Schedule an appointment.  Please use this link to request an appointment and someone will reach out to you shortly.  Select “Booster COVID-19 Vaccine Appt”.

  1. Walk into your healthcare center.  Please call us before you walk in, so we can make sure we have vaccine in stock that day and advise on possible wait time.
  2. Get the vaccine at any other licensed healthcare facility/pharmacy.  Please let your provider know, so we can add the information to your health record.
